| When a patient is diagnosed with a life-limiting or life-threatening illness, it is appropriate to consider hospice care. The patient and their family can discuss hospice with their family physician, healthcare professionals or the professional staff at Faith Hospice. If your physician is not familiar with the services and programs available through hospice, our medical director is ready to discuss the situation with him or her directly.
Admission to the Hospice Program
Once hospice care has been decided upon, a nurse from our admissions department will contact the patient's physician to verify that the patient is hospice appropriate. The admission is also reviewed by one of our medical directors. The patient will be required to sign informed consent and insurance forms that state that the patient understands that curative care will no longer be sought. Medicare patients sign a form that explains how electing the Medicare Hospice benefit affects other Medicare coverage.
Paying for Hospice Care
Hospice care is covered by Medicare/Medicaid, many insurance companies, HMOs, PPOs and managed care groups. Our staff can assist you in determining your hospice benefits.
