Hospice care can be delivered in a number of ways—in the home, in an assisted living facility, nursing home or hospital or in a hospice residence. Faith Hospice is able to provide all of these alternatives. Hospice care is supplemental care and not meant to take the place of a family member(s) caring for their loved one.
In the Home
Faith Hospice can provide hospice care directly in the patient's own home through regular visits by our hospice team. Our physicians direct a plan of care through home visits and collaboration with the hospice team.
- Our registered nurses (RNs) provide primary care with a focus on monitoring patient status and promoting comfort and symptom control. They are available 24 hours per day, seven days per week as needed.
- Our experienced Licensed Practical Nurses (LPNs) and Certified Nurses Aides (CNAs) are available to assist with the patient's needs as well as to assure comfort and symptom control.
- The Faith Hospice Nurses Aides and Certified Home Health Aides concentrate on personal care including help with bathing, toileting, eating, etc. We also have a trained group of dedicated volunteers available to run errands, perform light housekeeping or provide respite care for caregivers as needed.
- Finally, physical, occupational and speech therapists are available as determined by the hospice team. Our social workers provide emotional support to our patients and families and can address financial, insurance and legal issues. They also provide emotional support as families move through the grieving process. Our chaplains lead the spiritual care planning process by assessing the spiritual needs of the patient and family.
In Assisted Living or Nursing Facilities or Hospitals
Our Faith Hospice team is able to deliver hospice care wherever needed. We will work with your current care team to design a program that will provide optimum comfort and quality of life.
In a Hospice Residence
Sometimes it is not possible to provide hospice care in the patient's home, and it is necessary to consider a hospice residence. Our residence, Trillium Woods, is family-centered and dedicated to providing the patient with maximum comfort and enhancement of life. Family members and friends are encouraged to spend as much time as possible with the patient.
A hospice residence can also provide temporary, general inpatient care during a period of crisis—when pain and symptoms are out of control and need to be managed more closely. |
