  Faith Hospice Care Team
Hospice care is provided, in conjunction with the patient's primary physician, by an inter-disciplinary team guided by our medical directors.

Personal Physician
The patient's personal physician works with the family and our medical directors to create a plan of care.

Faith Hospice Medical Directors
Faith Hospice has medical directors on staff who are board-certified in hospice and palliative care medicine. Together with our nursing staff, they work to manage the patient's symptoms and ensure that the patient remains pain-free.

Registered Nurses
Registered nurses (RNs) provide primary care for the patient and family. They are available 24 hours per day, seven days per week, either through home visits or at Trillium Woods. Nurses monitor patient status and administer medications to promote comfort and symptom control. Our nurses work in conjunction with other staff members to identify and meet patient and family needs.

Licensed Practical Nurses and Certified Nurse's Aides
Our experienced LPNs and CNAs are an essential part of our hospice team whether serving at Trillium Woods or in the patient's home. They assist with the patient's personal needs as well as help to promote comfort and symptom control.

Nurse Aides and Certified Home Health Aides
These staff members assist with the patient's personal care.

Social Workers
Our social workers provide emotional support for the patient and family and can help resolve issues surrounding finances, funeral arrangements, etc. Our social workers continue to provide emotional support to the family as they work through the bereavement process.

Spiritual Staff
Our Faith Hospice chaplains are available to all patients and their families. Clergy from all faiths are welcome and encouraged to visit. A chapel is available at Trillium Woods for prayer or meditation.

Therapists
Physical, occupational and speech therapy is available as needed to help the patient maintain the highest possible levels of function and comfort.

Volunteers
Our trained volunteers are an important part of our hospice program offering companionship, support and assistance to patients and their families. They may run errands, do light housekeeping or provide respite for caregivers.

Bereavement Counselors
Grieving for the loss of our loved ones begins even before the actual time of death. Anticipatory grief is the normal mourning that occurs when a patient or family is expecting a death. Our bereavement counselors are a critical part of our team providing individual or group support to families before and after the death of their loved one.

Caregivers Support Groups
Care giving is an emotionally and physically draining experience. Our Faith Hospice team can provide coping strategies to help families face the challenge of caring for a dying member.
